What Are my Rights as a Patient?
 
As a Patient I have the right to :
 
» choose my care providers.
» expect care that is considerate and respectful.
» care that is appropriate and professional .
»
receive information necessary to give informed consent prior to the start of any treatment or procedure.
» be advised of any change in the plan of care, before the change is made.
»
refuse treatment within the confines of the law and to be informed of the consequences of his or her action.
»
voice grievances and suggest changes in service or staff without fear of restraint or discrimination.
» be informed of what to do in the event of an emergency.
» voice grievances without fear of discrimination or reprisal for having done so.
 
 
What Are my Responsiblities as a Patient?
 
 With my rights come, my responsibilities , and they are :
 
»
Provide complete and accurate information about your health, including present condition, past illnesses, hospitalizations, medications, natural products and vitamins, and any other matters that pertain to your health.
»
Provide complete and accurate information including your full name, address, home telephone number, date of birth, and employer when it is necessary.
» Reporting changes in your condition or symptoms, including pain, to a member of the physician team.
»
Informing your doctor or nurse when you are having pain and asking for pain relief measures when pain or discomfort first begins or is not relieved by prescribed treatment measures.
»
Following the instructions and advice of your treating doctor. If you refuse treatment or do not follow the instructions or advice, you must accept the consequences of your decisions.
»
Notifying a member of the health care team if you do not understand information about your care or treatment.
